1. Proof of age: Birth certificate.
2. Proof of identity: Recent close-up picture.
3. Proof of address: Parent's state ID, drivers license, or recent utility bill.
4. Proof of insurance: Current health insurance card.
5. Proof of health: Completed health form signed by doctor.

Includes round-trip transportations, 16 meals, daily snacks, lodging in air-conditioned rooms, and all activities.

Camp Lifeline
is a Christian overnight summer camp with the mission to see every camper choose to be transformed into the image of Jesus Christ, and empowered to be a changemaker in their communities for the Kingdom of God.
Our staff is comprised of dedicated Christian youth and adults who volunteer their time. Our staff attend mandatory training sessions and have passed background checks. Campers are supervised 24 hours a day by counselors who are positive role models and whose primary concern is the campers.
It is a program of ImageMakers Ministries, a 501(c)3 nonprofit organization.
